A new patient's first appointment: opening the record properly from day one
A badly opened record costs you for years: duplicates, wrong phone numbers, forgotten history. Here is how to get a new patient's welcome right, from booking to the end of the first consultation.
A new patient walks through the door. Within a few minutes you will determine the quality of everything that follows: how reliable their record is, how easily they can be reminded, how precise their history will be. Yet this is often the most rushed moment of the day, because the waiting room is full and « we'll complete it later ». That « later » almost never comes. Getting the first appointment right means investing once so you stop losing time afterwards.
What is at stake at the first visit
The first visit durably fixes three things. First, the patient's identity: an approximate spelling of the name or a wrong phone number will create duplicates and make reminders useless. Second, their history: what you did not ask on day one is often learned only after an incident. Third, the impression they keep of your organization: a smooth welcome, where nobody asks the same thing three times, builds trust before the consultation even starts.
Before the visit: prepare the ground
It all starts when the appointment is booked. When a new patient calls, it is the right moment to collect the essentials without overwhelming them.
- First name, last name, date of birth and mobile number, checked by repeating them out loud.
- The reason for the visit, to book a slot of the right length: a first appointment usually takes longer than a follow-up.
- The list of documents to bring: previous prescriptions, X-rays, lab results, health booklet, reports.
- A request to arrive slightly early, so that creating the record does not eat into the consultation.
The reminder sent the day before is a chance to repeat these instructions. A patient who knows what to bring arrives prepared, and the practitioner has the useful elements from the very first minute.
At reception: create the record once, and do it well
Welcoming a new patient follows a short routine, identical for the whole team. Before creating a file, check that it does not already exist: a patient coming « for the first time » may have visited two years ago, or been seen under a relative's name. That is the first cause of duplicates.
- Search first by name, date of birth and phone number before creating anything.
- Enter full civil details and a reachable mobile number, the one that will be used for reminders.
- Note the contact of someone to call when the patient is a minor or vulnerable.
- Scan the documents they bring on the spot and attach them to the record, rather than handing them back without a copy.
- Explain in one sentence how the practice works: SMS reminder, confirmation, opening hours.
A record is created once but read for years: every minute saved at opening is saved again at every later visit.
During the consultation: collect history without an interrogation
The practitioner reconstructs the history during the interview: personal and family background, allergies, current treatments, past procedures. What matters is that this information is written where it will be read again, not kept in memory. A background note scribbled on scrap paper no longer exists at the next visit, or when a colleague covers for the practitioner.
Structuring this input saves precious time: the same interview outline for every new patient avoids forgetting an important question on busy days, and makes records comparable with one another.
After the visit: close the loop
The first consultation does not end when the patient leaves the room. Before they go, several points remain that condition what comes next.
- The next appointment is booked and entered in the schedule before departure, with its reason.
- Procedures performed are invoiced, or a quote is handed over if care is planned, with fees stated clearly.
- The prescription, requested tests and expected results are attached to the record.
- The patient knows how and when they will be reminded, and what to bring next time.
This is what turns a one-off visit into a followed patient: someone who leaves with an appointment, a clear explanation and the feeling of having been expected is more likely to return.
Common mistakes to avoid
- Creating a file in a hurry « to go faster », without checking whether the patient already exists.
- Settling for a relative's phone number without noting whose it is.
- Handing documents back without having scanned them.
- Postponing the recording of medical history to the second visit.
- Letting the patient leave with no next appointment and no information on fees.
