Last-minute cancellations and reschedules: how to keep your schedule full
A patient who cancels at the last minute means a lost slot and an unbalanced waiting room. Here is how to handle cancellations and reschedules so your schedule stays full, without friction with your patients.
Every practice knows the scene: at 8:30 a.m., a patient calls to say they will not make their 9:00 appointment. The slot is lost, the secretary jots the news on a scrap of paper, and the doctor waits ten minutes before the next patient. Taken one by one, these incidents look minor. Repeated every week, they weigh on revenue, on team fatigue and on the quality of the welcome.
The good news: much of this loss is fixed by organisation, not by strictness. Here is a simple method you can apply starting tomorrow.
Cancellation, reschedule, no-show: three situations to tell apart
They are often lumped together, yet each calls for a different response:
- A cancellation announced in advance: the slot can be offered to someone else, the easiest case to handle.
- A reschedule: the patient wants to come, but on another day. You need to offer an alternative quickly, otherwise they drop out of follow-up.
- A no-show without warning: the slot is lost, and this is the one to reduce upstream.
Make cancelling easy for the patient
Paradoxically, the easier it is to cancel, the fewer silent no-shows you get. A patient who hesitates to call « so as not to disturb » often prefers to say nothing. It is better to give them a light, comfortable way to let you know, for example by replying to a reminder SMS.
The goal is not to discourage cancellations, but to receive them early, while the slot can still be reused.
Prevent rather than endure: the reminder the day before
The best lever remains the reminder. An SMS sent the day before recalls the date and time and, above all, invites the patient to confirm or flag a conflict. You learn the day before, not the same morning, that a slot is about to open up.
A cancellation known the day before is an opportunity; the same cancellation known at appointment time is a loss.
Filling the freed slot
When a slot opens, three habits help recover it:
- Keep a short list of patients willing to come earlier: those who asked for a closer appointment or who live nearby.
- Move an existing appointment forward when the patient benefits, which also rebalances the day.
- Use the slot for short consultations or follow-up checks, easier to slot in at the last minute.
For this to work, the team must see at a glance what is free. A visual schedule that you reorganise by simple drag and drop saves precious time compared with a crossed-out notebook.
Set a clear rule, and communicate it
Many conflicts come from a rule that was never stated. Without any harshness, you can post and repeat a simple guideline: let us know as early as possible if something comes up, so the place can go to another patient. Said kindly, it is almost always well received.
Keep a trace too: noticing that a patient regularly misses appointments lets you discuss it calmly with them, or adapt how you schedule them, for example by offering earlier-in-the-day slots.
Measure to improve
Without figures, it is hard to know whether your efforts pay off. Simply track, month after month, how many appointments are cancelled, rescheduled or missed, and when in the week they cluster. A pattern often emerges: a given day, a given time range, a given type of consultation. You can then act where it really matters.
How Uli helps you hold the course
Uli brings together on a single platform appointments (drag-and-drop schedule, real-time queue, SMS reminders the day before with confirmation by reply SMS), the patient record with its attachments, billing and unpaid-balance tracking. The confirmations you receive let you spot early which slots to reopen, and reorganise the day in a few gestures.
